Terms and conditions of the gift voucher

Regulations of the gift voucher/gift card/gift certificate issued by MEDflow Ltd.

§1 Glossary of terms of the regulations

The issuer of the gift voucher is MEDflow Sp. z o.o., 9 Nowina Street, 60-589 Poznań, NIP: 7831683152, hereinafter referred to as MEDflow or MEDflow Imaging Diagnostic Center or the Organizer.
A gift voucher, also known as a voucher, gift card or gift certificate, is a document issued to the bearer and entitling its user to redeem it at MEDflow Imaging Diagnostic Center, located at 9 Nowina Street in Poznan. The bearer of the voucher has the opportunity to use the document when using the services offered by MEDflow. The range of services is available on the website  medflow.co.uk/price list. A gift voucher is not: a payment card, an electronic payment instrument, an electronic money instrument.
The medical facility is MEDflow Imaging Diagnostic Center, operated by the Issuer of the gift voucher at 9 Nowina Street, 60-589 Poznań.
A customer is a person who purchases a gift voucher.
The amount of the gift voucher recharge is the value of not less than PLN 100 (in words: one hundred zlotys).
The user is the bearer of the gift voucher.

§2 General information

The Regulations specify the terms and conditions for issuing and redeeming a gift voucher. Regulations are available at MEDflow registration and on the website  medflow.co.uk/regulatory-voucher-gift.

§3 Issuance and expiration date of the voucher

The gift card is issued at the Exhibitor’s office during the opening hours of the medical facility. The condition for issuance is payment at the medical facility of the amount of the voucher credit. Payment can be made in cash, by credit card or via BLIK.
When ordering electronically, payment must be made to Bank Santander account number: 02 1090 1346 0000 0001 5695 8099 and send confirmation by email to: kontakt@medflow.pl.
Gift voucher is disposable. The validity of the gift voucher is no more than 3 months from the date of its issuance. Each time, the amount of power supply along with the expiration date is indicated by the Exhibitor’s staff on the voucher.

§4 Rules of use of the voucher

A Gift Voucher entitles its User to pay for services offered by the Exhibitor. The amount of the credit is a prepayment made by the Customer before the Gift Voucher is issued for the purchase and use by the User of the Exhibitor’s services offered on the website.  medflow.co.uk/pricelist.
The gift voucher is redeemed by giving or presenting it to the Exhibitor’s staff and declaring the desire to use this form of settlement before paying for the service.

When the value of services, realized by the voucher User, exceeds the current value of funds accumulated on the gift voucher, the User is obliged to pay the difference between the price of services and the value of the voucher. If the voucher is presented, the Exhibitor will annotate the voucher accordingly.
When the price of the services purchased with the gift voucher is lower than the amount of the credit, the User is not entitled to receive a refund of the resulting difference. In this case, the Exhibitor’s staff updates the voucher by adding a note on the User’s voucher about the current balance, i.e. the remaining recharge amount.

§5 Right to refuse to redeem a gift voucher

The issuer will refuse to redeem the voucher if:
1. the submitted voucher is not authentic, i.e., it has signs of having been tampered with, forged, or is materially damaged,

2. there are contraindications to the test, endangering life or health, or that may adversely affect the operation of the Exhibitor’s apparatus,

(3) The user does not have a referral for examination, if it is necessary to perform the examination, in accordance with applicable law,

4. there are insufficient funds on the voucher to purchase the service chosen by the User, in a situation where the User refuses or does not have the ability to pay the missing part of the price of the service at the moment,

5. the expiration date written on the gift voucher has passed.

§6 Refund of the voucher recharge amount

The amount of credit found on the gift voucher is not subject to interest and refund to the User or Customer, in particular, the gift voucher is not exchangeable for cash (in whole or in part), and cannot be returned to the Exhibitor or exchanged for other vouchers issued by the Exhibitor. Services purchased at MEDflow using a gift voucher are refundable under the general rules of law in Poland.

§7 Complaints

Comments regarding the issuance and redemption of the gift voucher can be submitted directly to MEDflow during opening hours or via e-mail: kontakt@medflow.pl. In the complaint, the contact details of the complainant should be provided to enable a response and a description of the event giving rise to the complaint. MEDflow will respond to the complaint within 14 days of receiving the complaint.

§8 Final provisions of the regulations

The Rules and Regulations may be amended by the Organizer at any time. The Organizer will notify the Customer or User of the changes by making the updated version of the Regulations available at medflow.pl/voucher-gift-voucher.The purchaser of a gift voucher, as well as the bearer of a gift voucher, declares that he or she has become familiar with the content of these regulations, accepts their content in full and undertakes to abide by their provisions. Purchase of a gift voucher is tantamount to acceptance of these regulations.
